In consideration for being allowed to participate in Guam Tennis activities, including but not limited to tournaments, matches, practices, and events, I, the undersigned participant, hereby acknowledge and agree to the following:
- I understand that tennis activities involve inherent risks, including but not limited to the risk of injury, accidents, and property damage. I voluntarily choose to participate in these activities, and I assume all risks associated with such participation.
- I understand that Guam Tennis and its organizers, coaches, volunteers, and facilities may not be held responsible for any injury, loss, or damage that may arise from my participation in these activities.
- I release Guam Tennis and all its affiliates, including their officers, directors, employees, agents, representatives, and sponsors, from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that I may sustain as a result of participating in these activities, including those caused by negligence.
- I agree to abide by all rules, regulations, and codes of conduct set forth by Guam Tennis. I understand that failure to comply with these guidelines may result in my immediate dismissal from the activities without any refund or compensation.
- I consent to receive medical treatment that may be deemed necessary in the event of an injury or illness during my participation in Guam Tennis activities. I understand that I will be solely responsible for any medical expenses incurred.
- I grant Guam Tennis and its authorized representatives the right to use any photographs, video recordings, or other media taken during my participation for promotional and educational purposes.
